Start a set and tap the screen once for every rep. Get 20 push-ups or squats in one set, or 5 pull-ups, and you move up a level.

No. During setup you pick the hardest version you can do cleanly for the full reps, and you start there. You can also skip a level whenever it's too easy.
Every level has form photos, setup steps and cues, and most have a looping video. The hardest levels carry a warning about the joints they load.
No. There is nothing to sign up for. Everything you log stays on your phone.
